Booking an Appointment Online
Tristar Medical group uses Health Search and Health Engine for online appointments, which allows you to book an appointment at any time, 24hrs a day.
If you are booking online please remember to book a longer appointment (see guide below) if you require a procedure, health assessment, care plan or have multiple concerns.

How to Register
You can download the form below and bring it with you to your first appointment. Please arrive 15 minutes prior to your appointment.

Tristar Medical Group clinics run a zero-tolerance policy and will not tolerate abuse of any kind.
If you need to speak to your doctor you may telephone the clinic. Each doctor has a designated time each day for returning telephone calls and will endeavour to call back. Alternatively, there may be another staff member who would be able to assist you
Transferring from another clinic
New patients should be willing to transfer their medical records from their previous doctor before they can be registered as a regular patient.
Please ask reception staff for a copy of this form.
In most circumstances, your GP will need to see you before prescribing repeat prescriptions. Please contact your local Tristar clinic for further details.
Please make an appointment to see your doctor for specialist referrals. Referrals to specialists will not be provided without prior consultation.
You must see a doctor prior to any medical certificates being issued.
Patients are asked to make a follow-up appointment to review their test results.

If you are unable to attend your appointment, please call, preferably at least 4 hours before your appointed time so we can re-allocate your appointment.
